As an RN Case Manager, you will work with an LPN to manage and provide care to critical patients in their homes. Our team meets the goals of our patients to work to get back to baseline or better. Critical thinking skills and all levels of nursing skills are performed daily.
The ideal candidate will desire the ability to support one patient from admission to discharge, ensuring a positive outcome. RN’s work with exceptional nurses, therapists, staff medical social workers, and phone triage nurses to truly provide the ultimate inpatient care. Job Summary: Provides services requiring nursing skills in accordance with the plan of care. Develops and coordinates the plan of care and provides case management. Provide observation / assessments and monitoring of health status, health problems, or affected health systems as appropriate to individual patients and as ordered by MD. Monitoring, directing, and supervising of LPNs and HHA staff members. Work with office personnel to ensure proper care plans are carried out for each patient including: schedulers, medical records, etc. Make appropriate communications (generally by telephone) as needed to MD’s, pharmacies, supply companies, and etc.
– Valid Nebraska and/or Iowa RN license
– Current CPR Certification
– 2-3 years (plus) of nursing experience
– Valid Drivers License
– Positive mental attitude
– Exceptional time management
– Ability to prioritize
Pay: $30.00 – $35.00 per hour
